Four ambulance staff and seven hospital workers have been suspended over claims they did not rush to help a man who died after collapsing outside an A&E.
The suspensions followed an incident on the site of Walsall Manor Hospital on 23 June, in which a patient died, the ambulance service said.
A man admitted with chest pains collapsed after going outside the A&E to get fresh air, local reports say.
Walsall Healthcare NHS Trust and the ambulance service are investigating.
The trust confirmed seven staff members, including nurses and porters, had been suspended while the matter was investigated.
Sue Hartley, director of nursing at Walsall Health Care NUTS Trust, said: "We are carrying out a full and detailed investigation and I can confirm that members of staff are suspended at the moment while this investigation is taking place."
Meanwhile, a West Midlands Ambulance Service spokesman said: "I can confirm that an incident happened on the Walsall Manor Hospital site, on Saturday, 23rd June in which a patient died.
"Four members of staff have been suspended and a full investigation has been launched by the trust which is expected to be completed within the next two weeks."
